What Is PECOS Enrollment?
A PECOS enrollment service helps eligible healthcare providers submit and maintain Medicare enrollment information online. PECOS stands for Provider Enrollment, Chain, and Ownership System—the CMS platform for managing provider and supplier enrollment records.
The system collects application information and supporting documents. Your Medicare enrollment contractor reviews the submission; creating an account or saving an application does not establish approval.
PECOS Application Processing, Reassignment of Benefits & Revalidation Services
A first-time applicant, an enrolled physician joining a group, and a practice updating its address need different actions. Codixa RCM reviews the existing record before starting work. This helps your team avoid duplicate applications and identify the transaction your situation requires.
PECOS Account Access and Existing Record Review
We help identify the CMS Identity & Access Management, or I&A, permissions needed for the assignment. For organizations, the appropriate officials must approve access relationships. A missing enrollment on your screen may require an access check or contractor assistance before another application begins.
Initial PECOS Provider Enrollment
We review your provider type, National Provider Identifier, business structure, locations, and intended billing arrangement. An NPI identifies the provider but does not establish Medicare enrollment. We then prepare the applicable enrollment pathway and identify the documents and signatures still needed.
Group Enrollment and Reassignment of Benefits
A practitioner joining a group may need a reassignment allowing that organization to receive Medicare payments for eligible services. We check the individual enrollment, receiving organization, and reassignment details together so your team can identify an incomplete connection before relying on the arrangement.
PECOS Revalidation and Enrollment Updates
We review contractor notices and existing records to distinguish revalidation from a change of information. Address updates, ownership changes, and other enrollment events need their own assessment. We identify the appropriate action and supporting records rather than waiting for the next renewal cycle.
Incomplete Application and Contractor Request Support
We review each request for missing documents, unclear information, or corrections against its stated deadline. Codixa RCM records the requested item, responsible person, and response status. If the contractor has rejected or denied the application, we review that notice before identifying next steps.

- Our Process
How Codixa RCM Manages the PECOS Web Portal & Electronic Signatures
An application needs accurate information, required signatures, and a documented submission. Our process checks those elements separately, then tracks contractor requests through review. Your team can see which steps are complete and which still need action.
Enrollment Assessment and Document Collection
We confirm your enrollment history, provider type, practice structure, and target start date. We then request application-specific records, such as licenses, tax documentation, ownership information, and banking evidence. For example, an organization’s legal business name should match its IRS record.
PECOS Preparation and Signature Coordination
We compare application entries with the supporting records and flag conflicts before submission. The provider or appropriate organizational representative completes the required signature steps. We retain the submission confirmation and tracking number, then check for outstanding documentation or signature requirements.
Contractor Follow-Up and Approval Handoff
We track requests, organize responses, and contact the enrollment contractor when clarification is needed. After approval, we review the notice, enrollment details, applicable dates, and reassignment information with your practice. Your billing team receives the records needed to assess enrollment-related claim requirements.

PECOS Medicare Revalidation & Enrollment Support
Useful feedback identifies the original application problem, the assistance provided, and the documented outcome. Details such as resolving an access issue or correcting a group reassignment help practices assess whether the experience matches their needs.
“When we expanded our multi-specialty clinic and brought on three new physicians, the credentialing paperwork became a massive bottleneck. We were losing thousands of dollars in billable time waiting on payer approvals. Codixa RCM stepped in and completely took over. They got our new doctors fully credentialed and enrolled with Medicare and all our major commercial payers in a fraction of the time it used to take us in-house. Their weekly reporting is transparent, and they simply don’t let applications stall. I highly recommend them to any growing practice.”
Sarah T.
Practice Administrator, Advanced Family Medicine
“As a solo practitioner, I was spending up to 15 hours a week chasing down denied claims, correcting coding errors, and dealing with billing headaches instead of focusing on patient care. Since outsourcing our full revenue cycle to Codixa RCM, my claim denial rate has dropped to under 3%, and our accounts receivable is the healthiest it has been in five years. They even handled my recent CAQH re-attestations seamlessly. Partnering with Codixa RCM gave me the peace of mind to just be a doctor again.”
Dr. Marcus Reynolds
MD, Internal Medicine
“Navigating collaborative agreements for our Nurse Practitioners and managing complex payer matrices was constantly stalling our clinic’s growth. We partnered with Codixa RCM for their credentialing maintenance and APP enrollment services, and it has been a game-changer for our operations. They track all our state licenses and expirables, proactively manage our Medicare revalidations, and their billing team ensures our submissions are clean on the first pass. They don’t just feel like a vendor; they operate like a true extension of our team.”

FAQs About PECOS Enrollment Services & CMS Web Portals
These answers address application timing, fees, account access, enrollment status, and ongoing record maintenance.
PECOS enrollment has no single approval timeline. Processing depends on the provider category, application type, screening requirements, and completeness of the file. Missing signatures or unanswered contractor requests can extend the review. Ask for a case-specific estimate and track outstanding requirements alongside the application’s submission date.
Service pricing depends on the providers, organizations, locations, and transactions involved. Company charges are separate from CMS application fees. Physicians, eligible non-physician practitioners, and their organizations generally do not pay the CMS enrollment application fee. Other provider categories should confirm whether a fee applies before submitting.
An existing enrollment may require an access review or help from your Medicare contractor. Check that you are using the correct account and approved organizational relationship. CMS directs users who cannot locate an existing enrollment to their contractor. Confirm the record’s status before assuming that a new application is necessary.
Your personal password should remain private. CMS provides access arrangements for approved users working on behalf of providers and organizations. Establish the appropriate relationship and permissions before work begins. Codixa RCM coordinates access requirements while the provider or authorized organizational representative retains responsibility for required reviews and signatures.
You can review application information in PECOS or use CMS’s application-status tool for supported submissions. Keep your tracking number and submission confirmation available. If the status does not explain an outstanding requirement, contact your enrollment contractor, which can answer application questions and provide information about the review.
A PECOS record alone does not establish every billing permission. Some eligible practitioners enroll solely to order or certify services, which differs from enrollment for billing their own services. Review the approved enrollment type, applicable dates, and any required group reassignment before relying on the record for claims.
Updating CAQH does not establish that your PECOS record has changed. The platforms serve different purposes, and Medicare enrollment updates require the applicable PECOS or CMS application process. When practice information changes, identify each affected record and retain separate confirmation that you completed the required update.
Medicare generally requires providers and suppliers to revalidate every five years, while DMEPOS suppliers generally revalidate every three years. CMS can also request an off-cycle review. Follow your actual due date and contractor notice, since missing revalidation may cause payment holds or deactivation of billing privileges.
